Please include the following:

1. Photo of work
2. Photo of signature
3. Photo of written title or inscriptions
4. Medium
5. Size
6. Owner’s Name
7. Owner’s address and telephone number.
8. Place and date acquired
9. Exhibitions, etc…
10. Any additional information
